This is a Contact Us Form

The form to your left is a "Contact Us" form. Whenever somebody fills it out, it will create a 'contact' record and a 'case' for you inside your system. To access this details of the contact or the case you will need to log in to your Admin Console and go to 'Customers > Cases'

To change the form i.e add some more fields to collect extra information you will need to go to 'Modules > Webforms' where you will find the 'Contact Form'. You can edit it and once you're finished, you will need to re-insert the form onto the 'Contact Page' to see the update
